এনজিওগ্রাম কি এবং কেন?

এনজিওগ্রাম রক্তনালির একটি পরীক্ষার নাম ও চিকিৎসাপদ্ধতি। এই পরীক্ষাটি এক ধরনের পরীক্ষার মতো। যার সাহায্যে হৃৎপিণ্ডের রক্তনালির, মস্তিষ্কের রক্তনালি, কিডনির রক্তনালি ও হাত-পায়ের রক্তনালি দেখা যায়। যেভাবে এই পরীক্ষা করা হয় সেই পদ্ধতিকে এনজিওগ্রাফি বলে এবং পরীক্ষার পর যেটি বের হয় তাকে বলে এনজিওগ্রাম। হৃৎপিণ্ডের রক্তনালির সমস্যা দেখার জন্য যে এনজিওগ্রাম করা হয় তাকে করনারি এনজিওগ্রাম বলা হয়। হৃৎপিণ্ডের একটি রোগ আছে, সেই রোগটির নাম করনারি আর্টারি ডিজিস বা ইসকেমিক হার্ট ডিজিস। কোনো কারণে যদি করনারি আর্টারির ভিতর চর্বি জমে হৃৎপিণ্ডের মাংসপেশির অক্সিজেন ও নিউট্রিশন সরবরাহে ব্যাহত করে, তখন বুকে ব্যথা হতে পারে। এসব রোগীর অবশ্যই করনারি এনজিওগ্রাম করা উচিত। যদি করনারি আর্টারিতে চর্বি জমে পুরোপুরি বন্ধ করে দেয়, তবে বুকে প্রচণ্ড ব্যথা অনুভব হবে। সঙ্গে সঙ্গে বমি হতে পারে। ঘাম হতে পারে ও বুক ধড়ফড় করতে পারে এবং শ্বাসকষ্ট হতে পারে। এই রোগকে তখন বলে থাকে। হৃৎপিণ্ডের মাংসপেশি যদি করনারি আর্টারিতে চর্বি জমার জন্য অক্সিজেন ও নিউট্রিশন না পায় তখন হৃৎপিণ্ডের মাংসপেশি শুকিয়ে যায় এবং হৃৎপিণ্ডের মাংসপেশি তখন চঁসঢ় করতে পারে না। ফলে শ্বাসকষ্ট হয়ে মৃত্যুও হতে পারে। সুতরাং Acute Myocardial Infarction হওয়ার আগে এনজিওগ্রাম করাটা বাঞ্ছনীয়। হৃদরোগ নির্ণয়ের জন্য ECG, Echo Cardiogram, ETT, 24 Hours Holter Monitor, Stress Echo-Cardiography, Thallium Scan অন্যতম। তবে আধুনিকতম চিকিৎসা পদ্ধতি হলে Coronary Angiogram (CAG) অনেক সময় ECG, Echo-Cardiography, ETT, Stress Echo-Cardiogram করে করনারি আর্টারি ডিজিস নির্ণয় করা সম্ভব হয় না। সেক্ষেত্রে করনারি এনজিওগ্রাম বা CAG করে রোগ নির্ণয় করা সম্ভব হয়। Echo Cardiogram করে হৃৎপিপণ্ডের মাংসপেশির রোগ এবং হৃৎপিণ্ডের কপাটিকার রোগ নির্ণয় করা যায়। হৃৎপিণ্ডের সব রোগের জন্যই এনজিওগ্রাম করা দরকার নেই। শুধু করনারি আর্টারি ডিজিস বা Ischaemic Heart Disease এর জন্য করা যেতে পারে ও চিকিৎসা নেওয়া যেতে পারে।   অনেক রোগী এনজিওগ্রাম করতে ভয় পান। এনজিওগ্রাম বুক কেটে বা বড় কোনো অপারেশন করে করা হয় না। ডান কুঁচকির একটা রক্তনালি আছে তার নাম ফেমরাল আর্টারি। লোকাল Anesthesia ব্যবহার করে Injection-এর একটি Needle দিয়ে ফেমরাল আর্টারিতে প্রবেশ করে, একটি ক্যাথেটার-এর মাধ্যমে সরাসরি হৃৎপিণ্ডের করনারি আর্টারি দেখা যায়।
